Welcome to Assumption of the Blessed Virgin Mary Catholic Church and our online Family
Faith Formation registration platform. If you are a returning family to our program, please
update your information. If you are a new family, welcome to our program! Please complete
this form and contact our office at (815) 634-4171 if you have any questions. Please Note that
the ID # requested on the application refers to your parish Envelope #. Sessions for 2025/2026
will be monthly with your choice of the Sunday morning session or the Wednesday evening
session. The program runs from September through April. The schedule of monthly dates will
be posted on Assumption's website and will also be sent to your email address.

Instructions
The registration fee is $150.00 for one child or $250.00 for two or more children. Payment is due at the time of registration. Please make your check payable to Assumption Church and send it to Assumption Church at 195 S. Kankakee Street, Coal City, IL 60416. You may also make your payment for Family Faith Formation through our online giving program. The link for online giving is https://www.osvhub.com/stmaryassumptionparish/giving/funds.

		If you are a newly registered family, please turn in your child's Baptism Certificate to the parish office.
